Elk Haven is a private nonprofit skilled nursing and rehabilitation center which provides a wide range of specialized health care services, including 24-hour professional nursing care and a comprehensive rehabilitative therapy program. Elk Haven is designed to provide comfort, safety and a homelike atmosphere for our residents. We have been providing nursing services to residents of Elk County and surrounding communities for over 35 years and are located close to the local hospital and emergency care services.

Silver Creek Terrace

Silver Creek Terrace is a beautiful nonprofit personal care community offering a personalized and supportive atmosphere for independent seniors. The emphasis of Silver Creek is to provide each resident help with their daily routine, such as dressing, bathing, grooming, dining and remembering to take medications. While independence is never hindered, the program at Silver Creek enhances self-esteem and encourages a fulfilling lifestyle. Silver Creek is located next to Elk Haven, allowing for transition between levels of care as resident's needs change.
Silver Creek Services
Per current guidance, select Infection Prevention measures are influenced by the SARS-CoV-2 (COVID-19) transmission rate in the community. Please check Elk County's current rate of community transmission (CDC COVID-19 County Check​) prior to your visit to determine what may be asked of you. 


When the transmission rate is HIGH, masks are required by all staff and visitors at all times. 

When the transmission rate is SUBSTANTIAL to LOW, masks are optional in most cases. 


As resident health and safety is important to us all, please remember the safest course at all times is to wear a mask upon entry to the facility and while walking throughout the building.
